O2 sim card failed please help?

hey i've had my phone a long time but the other night i tried intering my pin code and forgot it 3 times, then i entered my puk code and it said enter a new pin code and confirm it, but after i did that it said sim card failed and doesnt allow me do anything!!! what should i do? please help??
